O-DSMT

O-Desmethyltramadol (other names: Desmetramadol, O-DSMT, Mono-O-demethyltramadol) is an opioid substance belonging to the class of phenylpropylamine, a metabolite of tramadol, which has analgesic properties several times more active than tramadol.

History

Chemistry

(+/-)O-Desmethyltramadol is an atypical synthetic opioid, it contains two rings, it is a cyclohexane ring, it is connected to the R1 phenyl ring, substituted by R3 hydroxy group which is where and the cyclohexane ring is connected to the phenyl ring, it is connected to the dimethylamine group connected by a methyl bridge, has a substituent on the R2 cyclohexane ring.

O-Desmethyltramadol is produced as a racemate of its two isomers changing the direction of R2 and R1 bonds results in R- and S-enantiomers. The difference between O-DMST and tramadol is that there is no methyl group in the R3 methoxy substituent.

Pharmacology

This substance is relatively new and the pharmacological and toxicological properties are not well understood; the following is a summary of chemical studies and tests performed in rats.

O-DMST is an active μ-opioid agonist, more potent than the parent compound (tramadol), and is also a high-affinity ligand of δ- and κ-opioid receptors, unlike tramadol.

The (-)-O-DSMT enantiomer acts as an inhibitor of norepinephrine reuptake. Activation of μ-opioid receptors mimics endogenous endorphins, resulting in analgesia, euphoria, and anxiolytic effects.
